On August 9, 2019, Governor Kate Brown signed House Bill 2005 into law.  This new law establishes a paid family and medical leave program for all employees in Oregon beginning September 3, 2023.  There is a 1% tax that will begin September 3, 2023 (for FGSD employees).  Employees will pay 60% of the tax and employers will pay 40%.  You will see this new deduction (OR PFML) on your September paycheck.
 

1.  Contact The Standard Insurance Company at 866-756-8115 and reference the following:  Forest Grove School District, policy #762416
          • You should apply 30 days before your leave begins when you know about the leave in advance
          • You should apply within 30 days after your first absence if the leave is an unexpected emergency Applications filed after 30 days after the first day of your leave may be denied
2.  Notify HR as soon as possible that you will be out on an Oregon Paid Family Medical Leave3.  Fill out a Protected Leave form and send it to the HR office4.  Indicate on the Protected Leave form if you want to backfill your pay from The Standard with your sick leave and/or personal leave*

*Note:  all absences for an Oregon Paid Family Medical Leave (OR PFML) should be entered in Frontline as OR PFML Long Term or OR PFML Intermittent and pay will be docked from your paychecks while you are receiving pay from The Standard.  The law allows you to use your paid leave to make up the difference between your dock and the amount that you are paid from The Standard, but you MUST notify the payroll department in writing for this to happen (indicating that you want to do this on the Protected Leave form will suffice).
Note about PERS service credit
A PERS member cannot earn service credit for any month in which they are reported as on a leave without pay (OR PFML absences of 11 or more business days in a calendar month are reported as leave without pay).

Optional (voluntary) Insurance

Optional (voluntary) insurance such as Accidental Death & Dismemberment (AD&D) insurance, Group Life insurance, Short Term Disability (voluntary) insurance and Long-Term Care insurance are offered to all licensed employees.  The cost of any optional (voluntary) insurance is the responsibility of the employee.

*An employee electing to not enroll in optional (voluntary) Short Term Disability when initially eligible (when hired) will be subject to a late enrollment penalty upon enrollment.  This means that if you file a claim for any condition other than an accidental injury during the first 12 months your coverage is effective, Short Term Disability benefits will not become payable until after you have been continuously disabled for 90 days and remain disabled.
 
 

Flexible Spending Accounts (FSA)

You may enroll in Flexible Spending Accounts (FSA) during open enrollment.  If you don't re-enroll during open enrollment, existing flexible spending accounts will end September 30th.

403(b) Tax Sheltered Annuity (TSA)

403(b) Tax Sheltered Annuity (TSA) plans generally serve as a voluntary retirement savings designed to supplement retirement benefits available from State pension plans and Social Security. Go to the Carruth Compliance Consulting website for more information.
 
How do I set up a 403(b) account?
Review the approved vendor list and contact a financial planner or the vendor directly to set up a 403(b) Tax Sheltered Annuity (TSA), or 403(b) Roth Account. Once the account is active, submit a Salary Reduction Agreement to the payroll office to begin monthly contributions from your paycheck.
 
How can I make a change to an existing 403(b) contribution amount?
If you would like to make a change to an existing deduction, please complete a Salary Reduction Agreement and forward it to the payroll office.

Licensed Employees

Insurance Cap

Full time - The amount the district will pay toward insurance premiums (insurance cap) is $1,495.00 per month for 1.0 FTE licensed employees for the 2023-2024 plan year.  The Forest Grove Education Association (FGEA) will continue to pay up to $105.00 per month for 1.0 FTE licensed employees.  The total of the insurance cap and FGEA contribution is $1,600.00 per month for the 2023-2024 plan year.
Part time - The insurance cap and FGEA contributions listed above will be pro-rated for licensed employees employed between .50 FTE and full time. 
 

Opt Out Stipend

A licensed employee may receive an "opt out" stipend of $375.00 per month, or a prorated amount if working between .50 FTE and fulltime, if covered by another group medical plan (not an Individual Marketplace Plan, Oregon Health Plan, Medicaid or a Student Health Insurance Plan) and declines dental and vision insurance.  
 
Steps needed to obtain an Opt Out Stipend:
  1. Opt out of medical insurance in MyOEBB (you will need to enter information about your group medical coverage)
  2. Decline dental and vision insurance in MyOEBB
  3. You may enroll in or continue your enrollment in optional (voluntary) insurance(s)
  4. Complete an OPT OUT form and submit it to the payroll office by September 10, 2023.

Classified Employees

Insurance Cap

Part time - Classified employees that work between 25 and 34 hours per week are offered tiered rates for insurance (monthly rate is different based on who is covered).  The amount the school district will pay toward insurance premiums (insurance cap) for the 2023-2024 plan year is $730.00 per month.
Full time - Classified employees that work between 35 and 40 hours per week are offered composite rates for insurance (monthly rate is the same no matter who is covered).  The amount the school district will pay toward insurance premiums (insurance cap) for the 2023-2024 plan year is $1,460.00 per month.
 

Opt Out Stipend

A classified employee may receive an "opt out" stipend if covered by another group medical plan (not an Individual Marketplace Plan, Oregon Health Plan, Medicaid or a Student Health Insurance Plan) and declines dental and vision insurance.  
 
Part time classified (25 to 34 hours per week) are eligible for a $187.50 per month taxable stipend.
Full time classified (35 to 40 hours per week) are eligible for a $375.00 per month taxable stipend.
 
Steps needed to obtain an Opt Out Stipend:
  1. Opt out of medical insurance in MyOEBB (you will need to enter information about your group medical coverage)
  2. Decline dental and vision insurance in MyOEBB
  3. You may enroll in or continue your enrollment in optional (voluntary) insurance(s)

Administrative and Confidential /Supervisory Employees

Insurance Cap

All employees must submit information for a criminal background check.
 
If you are not a TSPC licensed employee, you must complete a fingerprint clearance.

1.  I-9 form
--Fill out section 1 except for your signature and the date which you will need to sign in the presence of someone in Human Resources office when you come to your onboarding appointment.
-- A list of acceptable documents can be found on the third page of this form.  Please bring the appropriate documents with you to your appointment.
If hired after January 1, 2020, you must fill out this new W-4 form
If hired after January 1, 2020, you must fill out this form as well as the Federal 2024 W-4 form
4.  EEOC form
This information is used for statistical and affirmative action purposes
Please attach a voided check or document from your bank with the routing number and account number
-- This information is used to verify your years of teaching experience for placement on the salary schedule.
-- Fill out one form for each previous school district (or private school) that you have worked for. 
-- It is your responsibility to send this form to your previous employer.
You must fill out this form if you are NOT a TSPC licensed employee.
8.  New Hire Meeting with Human Resources
You will be asked to set up a meeting with the HR Department to review your new hire forms, insurance benefits, and salary information before you begin teaching.  Please bring the following to this meeting:
 
  All of the above documents if you have not already emailed them
  Your Social Security card
  Appropriate documents for your I-9 form
  Transcripts if you have not already sent them
  Your selections for insurance benefits
